Clinical observations and guidelines note that these fluctuations can contribute to variable symptom control in patients using less frequent dosing schedules. This is when many patients feel their best, higher energy, improved mood, and stronger libido.
Testosterone therapy is sometimes combined with an aromatase inhibitor for men with secondary hypogonadism who wish to conceive children with their partners. Aromatase inhibitors like anastrozole prevent the conversion of testosterone into estradiol by aromatase. In addition to the prevention of testosterone conversion into DHT, 5α-reductase inhibitors also prevent the formation of neurosteroids like 3α-androstanediol from testosterone, and this may have neuropsychiatric consequences in some men. Androgens like testosterone are teratogens and are known to cause fetal harm, such as producing virilization and ambiguous genitalia. However, the association between testosterone supplementation and the development of prostate cancer is unproven.
Once you're certain that testosterone therapy is appropriate and safe, reduce the risk of infection by washing your hands and putting on clean gloves. Though the body naturally produces this hormone, it is sometimes artificially administered to treat certain medical conditions. Intramuscular injections are often done weekly, while subcutaneous injections are frequently administered twice weekly in smaller doses. Once a patient qualifies for testosterone therapy (1, 2), risks and benefits of therapy as well as pros and cons of each formulation should be discussed (see Table 1). Studies involving SC administration of testosterone cypionate or enanthate have used 1-mL Luer-Lok syringes with a 20- or 25-gauge 5/8-inch needle to inject testosterone into the SC tissue of the abdomen or thigh (28, 47). All patients should receive training from medical personnel on how to self-inject testosterone. Because studies of SC testosterone therapy are limited, this needs to be verified in future studies.
